In order to reserve a ride to Church, please call (240) 839-5740 or complete and submit this form at least 5 business days before the date of pick-up. If you require a recurring pick-up, please indicate on the form. Should your address has changed, please contact the JHDC CAR Coordinator as soon as possible. By completing this form I agree that I will not hold the Jesus House, DC or the JHDC CAR Volunteer liable for any claims or damages before, during or in any way connected with my travel to/from Church.
