Call-A-Ride Request Form

To request transportation to Church, please complete and submit this form at least 48 hours before your requested pick-up date. Transportation requests are fulfilled based on availability and must be for Sunday or Wednesday services at the church location, within a 20-mile radius.

If you require recurring transportation, please indicate this on the form. If your address or contact information changes, please contact the church office as soon as possible.

In order to reserve a ride to Church, please call (240) 839-5740 or complete and submit this form at least 5 business days before the date of pick-up. If you require a recurring pick-up, please indicate on the form. Should your address has changed, please contact the JHDC CAR Coordinator as soon as possible. By completing this form I agree that I will not hold the Jesus House, DC or the JHDC CAR Volunteer liable for any claims or damages before, during or in any way connected with my travel to/from Church.
